All of a sudden, after a HyperVM upgrade, I`ve been receiving :
mkdir: cannot create directory `/etc/sysconfig/network-scripts/bak': Read-only file system
Any ideas ?

Hello,
I have been having the exact same problem, I have had 3 different Hard Disk each time with a clean new OS.
Also i have now had a Complete new server, however every so often my server keeps going into Read Only Mode. Causing the server to become unusable and have to be rebooted.
Also 'dmesg' on the machine shows nothing bad at all and there is no kernel panic, when connecting to the server with a screen your just show a input and output error due to the Hard Disk being placed in read only mode.

Woo hoo !!! Remember my system went read-only ? After a reboot system was screwed really bad. DC has to do a journal fix ... it was a 2 way situation : either it would get fixed or screwed worse. I have 4 HDD in RAID5, still system got screwed because of the system file getting corrupted. Thinking of getting a 4 x 1TB SCSI RAID 10 ...
Also, reminder for all those that want VPSes : RAID 10 and off-server backups !!!

@dev : Me or Ashley ? Either way, how could I check ?
@ Ashley : I was lucky I guess to get the system online ( it`s pretty unstable at the moment ).
I signed up for an off-server backup solution ( cheap and very recommended on the hosting forums ). I`m rsyncing everything now. I need to backup everything, then, full reinstall ..
