First, CentOS 3 is not for 2.6 kernel, you'd better use CentOS 4. CentOS 3 might lack some stuff needed for 2.6 kernels, such as module-init-tools.

ata_piix is built-in OpenVZ kernel, that is why mkinitrd fails here. I think that more recent versions of mkinitrd does not fail - they just print warning instead. So upgrading to CentOS 4 might help here as well.

Finally, we are going to release vzkernel-install package with our own version of mkinitrd which is tailored to OpenVZ kernel.
