1. нет дело не в /etc/mtab на host. с ним все в порядке.
2. я настоятельно попросил бы Вас проапдейтить и ядро и vzctl/vzquota RPMs. в 056 ядре куча security дырок из mainstream.
3. теперь по существу. Когда в VPS включена user/group quota, vzctl на старте VPS'а исполняет скрипт:
/etc/dists/scripts/set_ugid_quota.sh
(или /etc/dists/scripts/vpsdistribution-set_ugid_quota.sh, если имеется).
В этом файле, если приглядется для 3-го runlevel создается service скрипт:
/etc/rc.d/rcX.d/S10vzquota

этот скрипт должен автоматически на старте VPS'а прописывать строку:
/dev/simfs / reiserfs rw,usrquota,grpquota 0 0
в /etc/mtab

так что, одно из двух, или vzctl не запускает set_ugid_quota.sh, или что-то не так внутри VPS'а.

Сможете дальше сами докопать?
