Hello,

I have been having the exact same problem, I have had 3 different Hard Disk each time with a clean new OS.

Also i have now had a Complete new server, however every so often my server keeps going into Read Only Mode. Causing the server to become unusable and have to be rebooted.

Also 'dmesg' on the machine shows nothing bad at all and there is no kernel panic, when connecting to the server with a screen your just show a input and output error due to the Hard Disk being placed in read only mode.
