Currently I have a customer have trouble install APF/BFD

Please, check failcounters in /proc/user_beancounters.

Does this file (lib/iptables/libipt_IN_SANITY.so) exist? Hmm... do you use OpenVZ/Virtuozzo?

Seems that IN_SANITY is a CHAIN, that does not exist, but should exist before using the rule in question.

Yes but the odd thing is this error shows up right from a new rebuild. Template issue?

The problem, I suppose, not in the template.

Somebody (BTW, who?) tries to add the rule, that is not legal.
So, the problem is in this entity, that adds illegal rule...

So basically they try to add a rule which is not available in the service and it gives them a error?

Not exactly. They try to add the rule, that jumps to IN_SANITY chain. But this chain is missing. So, I guess, they forget to run some preliminary rules, that add IN_SANITY chain.
