This normally means that an entry in /etc/inittab with 'respawn' action
exits too quickly. Mind posting your /etc/inittab?

Where is this line from? I don't remember off the top of my head any
config file in RHEL4 with this syntax...

Try to look at /var/log/messages and /var/log/secure.
