are these rules:
ACCEPT all -- 0.0.0.0/0 0.0.0.0/0
intentional?

If I'm not mistaken the above comes from:
$IPT -I INPUT 1 -p ALL -m state --state RELATED,ESTABLISHED -j ACCEPT
$IPT -I OUTPUT 1 -p ALL -m state --state RELATED,ESTABLISHED -j ACCEPT
# above 2 rules allow response to future rules using --state NEW
