Hi,
Looking at your original error output:

two thoughts:
- outdated perl - I run with v.5.8.8 (could explain /dev//)
- you do not have enough Free PEs in volume group. Run vgdisplay and check for "Free PE / Size". You should have at least 512Mb available. If you have less, you have to repartition the server. On my server I have:
